Pragmatic Free Trail Meta is an open data platform that enables research into pragmatic trials. Background
Pragmatic trials are increasingly recognized as providing real-world evidence for clinical decision making. However, the usage of the term "pragmatic" is not uniform and its definition and assessment requires further clarification. Pragmatic trials are intended to inform clinical practices and policy decisions rather than prove a physiological or clinical hypothesis. A pragmatic study should strive to be as close as it is to real-world clinical practices, including recruitment of participants, setting, designing, delivery and implementation of interventions, determining and analysis outcomes, and primary analysis. This is a major difference between explanatory trials, as defined by Schwartz & Lellouch1, which are designed to test a hypothesis in a more thorough way.
The most pragmatic trials should not blind participants or the clinicians. This can result in bias in the estimations of the effects of treatment. Pragmatic trials will also recruit patients from different healthcare settings to ensure that their results can be generalized to the real world.
Additionally the focus of pragmatic trials should be on outcomes that are crucial for patients, such as quality of life or functional recovery. This is particularly important when trials involve the use of invasive procedures or could have harmful adverse impacts. Despite these guidelines, many RCTs with features that challenge the concept of pragmatism have been mislabeled as pragmatic and published in journals of all kinds. This could lead to false claims about pragmatism, and the use of the term should be standardized. The development of a PRECIS-2 tool that can provide an objective and standardized evaluation of the pragmatic characteristics is a first step.
Methods
In a pragmatic trial the goal is to inform policy or clinical decisions by showing how an intervention could be integrated into everyday routine care. This differs from explanation trials, which test hypotheses about the cause-effect relationship in idealised situations. Therefore, pragmatic trials could be less reliable than explanatory trials, and could be more susceptible to bias in their design, conduct, and analysis. Despite these limitations, pragmatic trials may contribute valuable information to decision-making in the context of healthcare.
The PRECIS-2 tool measures the degree of pragmatism within an RCT by assessing it on 9 domains ranging from 1 (very explicative) to 5 (very pragmatic). In this study, the areas of recruitment, organisation and flexibility in delivery, flexible adherence, and follow-up were awarded high scores. However, the main outcome and method of missing data was scored below the pragmatic limit. This suggests that it is possible to design a trial that has excellent pragmatic features without harming the quality of the results.
It is, however, difficult to determine how pragmatic a particular trial is, since pragmatism is not a binary characteristic; certain aspects of a trial can be more pragmatic than others. More methods of a trial can be affected by changes to the protocol or logistics during the trial. Koppenaal and colleagues found that 36% of 89 pragmatic studies were placebo-controlled, or conducted prior to licensing. They also found that the majority were single-center. A common aspect of pragmatic research is that researchers attempt to make their findings more relevant by studying subgroups of the trial sample. However, this often leads to unbalanced comparisons and lower statistical power, which increases the risk of either not detecting or incorrectly detecting differences in the primary outcome. The use of these terms in titles and abstracts could suggest a greater awareness of the importance of pragmatism but it is unclear whether this is manifested in the content of the articles.
Conclusions
As the value of evidence from the real world becomes more popular the pragmatic trial has gained popularity in research. They are clinical trials randomized which compare real-world treatment options instead of experimental treatments under development, they include patients that are more similar to the patients who receive routine medical care, they utilize comparisons that are commonplace in practice (e.g., existing medications), and they depend on the self-reporting of participants about outcomes. This method can help overcome the limitations of observational research for example, the biases that come with the use of volunteers and the lack of coding variations in national registries.
